I hereby certify that I do not have any health concern that would affect my participation in activities at Thaiger Muay Thai, Inc.

I, the undersigned, understand that by signing this waiver I agree that:

  1. Allocation of Risk on Athlete I understand there are significant risks involved in all aspects of Muay Thai and full contact fighting in general that may result in serious bodily or psychological injury, permanent disability, paralysis, or death. I also understand that there is a risk of incurring some injuries that may not become apparent until sometime after the training or a fight, which may also result in permanent disability or death, and I:
  2. Willingly and voluntarily expose myself to the risks above;
  3. Accept full responsibility for any injury or death, whether caused by me or caused by the actions or inactions of other competitors, event host, Thaiger Muay Thai, Inc. (“TMT”) officials, staff and volunteers;
  4. Assume all responsibility for any losses, costs, and damages that may be incurred as a result of my participation in the event.

2. If at any point before or during my training or fight I believe conditions to be unsafe, I will immediately notify the TMT staff , or any other official, and if necessary discontinue further participation in the activity.

3. Drugs I promise not to consume and not to be under the influence of any drugs. I understand that if such substances or traces of such substances are found in my system during my training with TMT.,

4. In consideration of being allowed to participate in training,seminars or fights (inclusive of smokers), I hereby fully release TMT, their agents, employees, affiliates, an action that may arise, directly or indirectly, out of my participation in the event, as well as liability incurred as a result of negligent acts or omissions of the above parties.

5 I understand that by placing my initials in this section and signing the bottom of this document, I agree to indemnify TMT and the above parties from any liability for injury or death caused to any other person, we well as damage to the property, however caused by "TMT".

6. I understand that this release shall be binding upon me, my heirs, executors, administrators and successors.

8. Media I give TMT permission to use any pictures, video, portrayal of my likeness obtained during training,seminars and or fights, along with my name, fight record, and physical attributes, for promotional, educational and training purposes, or any other operational purpose TMT sees fit, without my prior express permission, and without compensation to me.

9. Severability and Entire Agreement This waiver is complete, and there were no representations made to me that contradict anything stated in this document. If any portion of this waiver is held invalid, the remainder of the waiver shall remain in force.

Rules & Regulations

The following conduct or behavior is not permitted at any time within Thaiger Muay Thai premises or group excursions:

  1. Disorderly behavior of any kind, including excessive noise, vulgar, cursing or otherwise causing disruption to the peace and order of the class. Be humble and respectful; treat others and you would want to be treated
  2. Absolutely no physical/verbal fighting and sexual activities will be tolerated within the vicinity of Thaiger Muay Thai.
  3. Harassment of other Thaiger Muay Thai guests or staff, either verbally or physically is prohibited. This may include verbal abuse, use of profanity or other abusive language, intimidations, staring at or following another person within the facility, sexual harassment or harassment on the amount of race, religion, ethnic background, gender or sexual orientation.
  4. Refrain from training without proper protection. This includes gloves while hitting the bags or pads; and all safety equipment must be worn will sparring (mouth guard, shin guards, cup, head gear, hand wraps and 14/16 Oz gloves)
  5. Sparring and Competion- Head Traininer's (Patrick Bozeman) permision is required for sparring and competition/fights of all types. 
  6. Vandalizing or defacing materials or property including equipment, furniture, walls or any other aspect of Thaiger Muay Thai is prohibited. Return all equipment once done (i.e. rental gloves, shin guards, weights, pads, skipping ropes, etc.).
  7. Removing materials, equipment or property from the building without authorization is prohibited.
  8. Being unhygienic before, during, and/or after training. Clean and wash all personal and gym equipment with a proper disinfectant (i.e. Gloves, shin guards, head gear, weights etc.)
  9. Failure to clean up after oneself while using any part of the facility, this includes re-racking weights when finishing a set in the free weights area may result in a ban from using this area.
  10. Indecent exposure, voyeurism, exhibitionism, or other lewd lascivious acts is prohibited.
  11. Training while being sick is highly ill-advised (we encourage optimal health for ALL the members)
  12. Being reckless on or inside the ring. Do not jump off the ropes, turn buckles or perform any dangerous stunts with the ring.
  13. Leaving trash or eating in the training area is prohibited
  14. Leaving personal property in the facility unattended. All property must be stored in a locker. Property left unattended will be confiscated or disposed of. Thaiger Muay Thai is not held responsible for any personal items and valuable belongings that go missing; please place items in locker or secure visible place. Locker use is for gym hours only; any locks that are left overnight without proper registration will be cut off.
  15. Bringing firearms or any other deadly weapons into Thaiger Muay Thai is strictly prohibited.
  16. Bringing a bicycle into the facility. No bicycles are to be left near any of the entrances.
  17. No rollerblades, skateboards, inline skates, scooters or any other sports equipment that may cause damage to our floors are allowed use in the facility.
  18. No sleeping at the gym without supervision from TMT staff.
  19. Possessing or consuming alcohol or illegal drugs or being under the influence of alcohol or illegal drugs while at Thaiger Muay Thai is prohibited.
  20. No smoking tobacco products including any type of electronic cigarette, hookah, etc.
  21. Selling products or services, soliciting for personal gain; or approaching guests for the purpose of obtaining signatures for petitions within the building is prohibited.
  22. Posting or distributing unauthorized material or literature is prohibited. Approval must be obtained in advance from TMT staff.
  23. Abandonment or individuals, both adults and children, who are unable to take care of themselves is prohibited. A responsible guardian must attend to children under 16. The responsible adult must be within arm’s length of their children at all times unless the child is participating in class.
  24. Membership must be paid on time at the beginning of every month or end of every month. Training packages do not roll over to the next month. 
  25. No overnight parking of bicycles. 

 

MEMORANDUM OF AGREEMENT

  1. I understand that Muay Thai and such other martial arts as the instructors and I include in the studies at Thaiger Muay Thai are martial arts; that is the systems of unarmed fighting
  2. I acknowledge, understand and agree that Thaiger Muay Thai activities and exercises are strenuous whether training individually or participating in classes with other members and instructors. By joining the exercises and activities I agree there is a possibility of physical illness, physical injury minimal, serious or catastrophic (including but not limited to abrasions, bruises, bleeding, broken bones, cartilage or organ damage), emotional or psychological injury, death, or property damage when use on people or equipment in the academy; in addition, that the participant is assuming the risk by participating.
  3. I understand that the types of injuries may result not only from intentional actions, but also from inadvertent, negligent, or reckless action; performed by the instructor or others including the malfunction or failure of any of the equipment or facilities at Thaiger Muay Thai (including, but not limited to pads, protectors, punching bags, and mats); and errors or mistakes in the instruction or performance of fighting or defense techniques by instructors or other students at Thaiger Muay Thai.
  4. I understand that the risks of any and all of the types of injuries described above cannot be significantly reduced or eliminated without adversely affecting and even jeopardizing, the nature and quality of the instructions and activities at Thaiger Muay Thai.
  5. I further agree to hold Thaiger Muay Thai and its Owners, Directors, Staff, or other associated representatives including Thaiger Muay Thai's current insurrance policy holder and any other subsequent body from any and all liability for personal injury or property damage suffered on the grounds of the facilities. I also agree that Thaiger Muay Thai and its Owners, Directors, Staff, or other associated representatives including Thaiger Muay Thai's current insurance policy holder and any other subsequent body do not authorize me to use any of the fighting or defense techniques that I learn in my studies at Thaiger Muay Thai against any other person, in any circumstances other than a) martial arts training, matches, exhibitions, or demonstrations in which my instructors and I agree I am sufficiently qualified and physically able to participate at Thaiger Muay Thai or elsewhere; and b) self-defense or defense of others in a situation that involves the risk of imminent physical injury or death. Furthermore, I authorize the Owners, Directors, Staff, or other associated representatives of Thaiger Muay Thai to act for me, according to their judgement, in any emergency requiring medical attention in facilities. I hereby hold the Owners, Directors, Staff or other associated representatives harmless in the exercise of this authority.
  6. I, the parent or guardian, understand that I will be notified as soon as possible in the event of an emergency. All expenses of such treatment will be assumed by me or my insurance carrier.
  7. I knowingly and voluntarily agree to assume all of the risks associated as stated above and hold harmless, and waive any and all rights to initiate or maintain a lawsuit or otherwise seek or obtain any finding of liability against Thaiger Muay Thai and its Owners, Directors, Staff, or other associated representatives includingThaiger Muay Thai's current insurance policy holder and any other subsequent body.
  8. I understand that Muay Thai and such other martial arts as the instructors and I include in the studies at Thaiger Muay Thai are martial arts; that is the systems of unarmed fighting

  15. MEDIA/APPEARANCE CLAUSE: Permission is granted to use me or my child’s picture or image in any or all future advertisements, broadcastings, web site, marketing literature or promotional videos for Thaiger Muay Thai and/or events sponsored and conducted by TMT.

PHOTO RELEASE FORM

I grant permission to Thaiger Muay Thai, Inc. and its agents or employees, to use photographs taken of me or my minor child/children in classes or events of Thaiger Muay Thai, Inc. for use in publications such as brochures, newsletters, and magazines, and to use the photographs on display boards, and to use the photographs on display boards, and to use such photographs in electronic versions of the same publications or on Thaiger Muay Thai Inc. web sites or other electronic form or media, and to offer them for use or distribution in other non-company publications, electronic or otherwise, without notifying me.

I hereby waive any right to inspect or approve the finished photographs or printed or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the photograph.

I hereby agree to release, defend, and hold harmless Thaiger Muay Thai Inc. and its agents or employees, including any firm publishing and/or distributing the finished product in whole or in part, whether on paper or via electronic media, from and against any claims, damages or liability arising from or related to the use of the photographs, including but not limited to any misuse, distortion, blurring, alteration, optical illusion or use in composite form, either intentionally or otherwise, that may occur or be produced in taking, processing, reduction or production of the finished product, its publication or distribution.

  1. Recognize and understand that martial arts training is an activity that involves physical contact and that my participation might result in serious injury, including permanent disability or even death, and severe (social and economic loss).
  2. Recognize and understand that such risk may be due to not only my own actions, but also the action, inaction or negligence of others, the regulations of participation, or the conditions of the premises, or of any of the equipment used.
  3. Recognize that there may be other risks that are not known to me or to others or not reasonably foreseeable at this time.
  4. Agree to inspect the facilities, equipment and pairings prior to participation, I will immediately inform an instructor if I believe that anything is unsafe or beyond my capability and refuse to participate.
  5. Assume all of the foregoing risks and accept personal responsibility for any damages that may result from injury, permanent disability or death.
  6. Enter martial arts training and/or competition entirely of my own free will and understand the importance of following the rules of training and competition.
  7. I certify that I am in good physical condition, and have no disease, injury or other condition that would impair my performance or physical and mental well-being during intense training practice and/or competition.
  8. Grant permission in case of injury to have a doctor, nurse, athletic training or other emergency medical personnel provide me with medical assistance or treatment for such injury.
  9. Release, waive, discharge and covenant not to sue, Thaiger Muay Thai Inc., its affiliated organizations and governing bodies, their officers, instructors and personnel, other members of the organizations, participants, supervisors, coaches, sponsoring organizations or their agents, and if applicable, owners and leasers of the premises from any and all liability to the undersigned, his or her heirs and next of kin for any and all claims, demands, losses and damages which may be sustained and suffered on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the releases or otherwise.

I HAVE READ THE ABOVE WARNING, WAIVERS, RELEASE, AND ASSUMPTION OF RISK. I FULLY UNDERSTAND ITS CONTENTS, AND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT. I HEARBY SIGN IT VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE, OR GUARANTEE BEING MADE TO ME AND INTEND MY SIGNATURE TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY.
